India. Application of Hyperbaric Oxygen Therapy (HBOT) in Sports Medicine:

Hyperbaric Oxygen Therapy (HBOT) is gradually gaining popularity as an effective treatment option for a number of medical conditions. HBOT is used equally by common people as well as celebrities and sports-persons to boost their health and fitness.

The healing of an injury caused in sports has its own natural recovery process, and follows almost a constant pattern regardless of the cause beneath. This process constitutes 3 phases:

1. Inflammatory phase
2. Proliferative phase
3. Remodeling phase

The hypoxia-induced factor-1α has been found to be important in the inflammatory phase. The hypoxia-induced factor-1α promotes, for instance, angiogenesis, vascularization, and the glycolytic system. However, if we can control the oxygen supply without promoting the blood flow, it would be possible to control blood vessel permeability, to diminish swelling and thereby, sharp pain.

In proliferative phase, the supply of oxygen to the injured region is increased in musculoskeletal tissues, except cartilage. Oxygen is vital for the generation of extracellular matrix components like proteoglycan and fibronectin.

In remodelling phase, tissue replacement takes place gradually over many hours, employing the supply of oxygen provided by blood vessel built already into the musculoskeletal system organization, with exception of cartilage. In case the damage is minor, it's possible to recover the tissue with almost perfect organization. However, if the damage extent is large, a scar constituting mainly collagen, may substitute the tissue. Depending upon the injury, consequently, the collagen becomes hard or loose for muscle repair or ligament repair respectively.

HBOT application for treating sports injuries has been recently suggested as a therapy modality in scientific literature. A therapy modality is an adjunct or a primary treatment. Although the results are promising when it comes to using HBOT as treatment modality in case of sports-related wounds and injuries, however, the studies are limited because of small sample sizes, randomization problems and lack of blinding.

Even less number of studies can be seen in the literature that refer to the application of HBOT in case of high level athletes. Ishii and colleagues outlined that HBOT was used as a recovery therapy for muscular fatigue in Nagano Winter Olympics. In this experiment, HBOT treatment was provided to 7 Olympic athletes for 30-40 minutes at an atmospheric pressure of 1.3 ATA, with an average of 2 treatments per athlete and a maximum of 6. It was discovered that all the athletes benefited from Hyperbaric Oxygen Therapy, and the recovery rates were found to be faster. These results agreed with the results obtained by Haapaniemi and colleagues and Fischer and colleagues, which outlined that ammonia and lactic acid were removed faster through HBOT, resulting in shorter recovery periods.

Owing to the effectiveness of HBOT in healing sports injuries, more and more sportspersons and athletes are using it to recover from injuries and also to boost their overall health and fitness.

Sports professionals known to be using Hyperbaric Oxygen Therapy (HBOT):

Athletes need to be in great physical health in order to perform well. The risk of injury in any sport is high. More athletes are now becoming involved with their care, recovery and well being. One of the therapies they are choosing to use is Hyperbaric Oxygen Therapy.

Hyperbaric oxygen therapy is used as an adjunctive therapy to treat sports injuries, from the acute traumatic injury to muscle contusions and ankle sprains. It is also used in treatment of joint, ligament, and tendon injuries reducing recovery time. Hyperbaric oxygen helps with the reduction of swelling and facilitates soft tissue healing. Many professional sports teams are now using hyperbaric oxygen therapy to attain their performance and to reduce fatigue.

Hyperbaric Oxygen Therapy helps to get athletes to get back to peak physical fitness following injuries and strains acquired during training sessions and competing in world events.

In the UK many rugby and foot ball players use hyperbaric oxygen therapy as an adjunct to other forms of therapy.

India. Hyperbaric Oxygen Therapy (HBOT) at a glance

Oxygen is a colorless, odorless gas that makes up about 21% of the atmosphere. It is essential for life for two main reasons:

1. Oxygen is one of the body’s basic building blocks. All of the body’s major components – water, protein, carbohydrate, and fat – contain oxygen.

2. Oxygen helps promote certain chemical reactions within the body that result in energy production. Energy is needed for proper circulation, respiration, digestion, temperature control, and many other important functions.

Nature has proven that healing cannot take place without appropriate oxygen levels in the body’s tissues. Many serious health problems stem from or are complicated by tissues having low oxygen levels. In many cases adequate oxygen cannot reach the damaged areas. Without adequate oxygen in the tissues, the body’s natural healing process fails to function properly, and we begin to see damaged tissues, decreased tissue function, slow healing, dry & sagging skin, aging,  fatigue, wrinkles,  and a variety of other symptoms.

Oxygen given under normal atmospheric pressure is not enough to raise tissue oxygen levels to reverse this lack of oxygen because red blood cells cannot carry and release enough extra oxygen. In order to overcome the oxygen starvation and raise tissue oxygen levels high enough for optimal healing to occur, the oxygen must be delivered under slightly increased atmospheric pressure.

Blood is made up of three main components: White blood cells that help fight infection; red blood cells that carry oxygen; and plasma, which is the fluid that carries both kinds of cells throughout the body.

Normally, only the red blood cells deliver oxygen, via a protein called hemoglobin, to the body’s tissues and cells. A healthy person’s hemoglobin is 97% saturated with oxygen when exposed in our lungs to normal air, or 100% saturated when breathing pure oxygen. Whether breathing air or pure oxygen, the limited number of red blood cells can only deliver a certain level of oxygen to the body’s tissues.

Hyperbaric Oxygen Therapy (HBOT) is a 300 year old medical treatment that consists of administering 100% oxygen at greater than atmospheric (sea level) pressure in order to improve and correct certain medical conditions. The underlying principle of hyperbaric oxygen therapy is quite simple: HBOT saturates the body and its tissues with oxygen so they can function properly.

When HBOT is used, you are breathing 10 times the regular amount of oxygen under increased pressure. These increased oxygen molecules are not only dissolved in the red blood cells, but also forced into the blood plasma, which makes up the bulk of the blood. This allows oxygen to be delivered to every parts of tissues and cells that  that may be deprived of oxygen.

In short, Hyperbaric Oxygen Therapy (HBOT) works by saturating a person’s blood and plasma with oxygen resulting in increased oxygen delivery to tissues. In fact, because HBOT forces oxygen into the body under pressure, oxygen dissolves into all of the body’s fluids including the blood plasma, the lymph, and the cerebrospinal fluid surrounding the brain and spinal cord. All of these fluids carry the extra oxygen to the tissues and cells of the body, even where circulation is poor or blocked.

Using HBOT to increase the oxygen level in body tissues produces several long-term therapeutic effects and facilitates healing through a number of mechanisms, such as:

• Growth of new blood vessels and capillaries to increase blood flow
• Relief of oxygen starvation (hypoxia)
• Enhancement of the immune system
• Reduction of free radicals that may continue to cause tissue damage
• Constriction of blood vessels (vasoconstriction) to decrease swelling & edema
• Protection of the integrity of cell membranes so they can function properly
• Stimulation of fibroblasts (cells involved in wound/tissue healing)
• Increasing collagen synthesis
• Increasing ability of white blood cells to destroy bacteria and remove toxins
• Decreasing the number of viruses in the blood
• Detoxification
